Trip Visit - Details about Trip (Date, Location, Who will be going, as MUCH as you can tell!) *

Party Visit - If this is a Party, tell us your theme and what kind of fun you have planned. Also include anything you would like The Fairly Godmother to do special (be there for candles on the cake, surprise entrance, pictures with each child separately, etc....keeping in mind, this on average is 40 minutes.) *

What do your kids WISH for? (Sports teams, toys, things that make them happy) If for a party, only the WISH of the featured child is needed. *

Name and Information about a special stuffed animal, sleepy toy/object, or at least a pet. We use this object to be the "portal" in which The Fairy Godmother hears about wishes/challenges....because, your not going to have little birds flying around your kitchen who will come back and tell us you need a glass slipper, now will you? *
